CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
Why this applies to Fluticasone Proprionate, Salmeterol
Fluticasone Proprionate, Salmeterol falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Delta-9-tetrahydrocannabinol (thc) interacts with.
In vitro research shows that cannabis containing THC might modestly inhibit the activity of CYP3A4 enzymes, which might decrease the metabolism of CYP3A4 substrates. It is unclear if this effect is due to THC, other constituents, or the combination. In vitro research also shows that cannabis extracts containing THC modestly inhibit the CYP3A4 metabolism of testosterone; extracts providing the specific cannabinoids cannabidiol (CBD) or cannabigerol (CBG) alone had stronger inhibitory effects than extracts containing both THC and CBD. Conversely, a crossover clinical study in healthy adults shows that oral THC does not inhibit CYP3A4.

Your AirDuo inhaler combines two medicines: fluticasone (a steroid that calms airway inflammation) and salmeterol (a long-acting bronchodilator that keeps your airways open). The fluticasone part is cleared by a liver enzyme called CYP3A4. The thinking here is that THC might slow that enzyme a little, which could let more fluticasone linger in your system.
Honestly, this is a very weak concern. The evidence comes only from lab dish studies, and a human study found oral THC did not actually block CYP3A4. If anything happened at all, it would be a slight bump in steroid exposure (think hoarseness, mouth thrush, or rarely steroid-type effects). For most people this is unlikely to matter, but since asthma control is important, it's worth a quick mention to your pharmacist or doctor.
