Why this applies to Macitentan, Tadalafil

Macitentan, Tadalafil falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Delta-9-tetrahydrocannabinol (thc) interacts with.

Theoretically, THC might increase the levels and adverse effects of CYP2C9 substrates.
In vitro research shows that THC moderately inhibits the CYP2C9-mediated 7-hydroxylation of S-warfarin in a concentration-dependent manner. In vitro research also shows that cannabis extracts containing THC modestly inhibit the CYP2C9 metabolism of tolbutamide; extracts providing the specific cannabinoids cannabidiol (CBD) or cannabigerol (CBG) alone had stronger inhibitory effects than extracts containing both THC and CBD. Theoretically, THC may inhibit the metabolism of other CYP2C9 substrates. Conversely, a crossover clinical study in healthy adults shows that oral THC does not inhibit CYP2C9.

Opsynvi combines two blood pressure and pulmonary artery medicines: macitentan and tadalafil. The idea here is that THC might slow down a liver enzyme called CYP2C9, which helps clear part of this medication. If that enzyme worked more slowly, the drug could build up a bit and its effects might feel stronger. For tadalafil that could mean more flushing, headache, dizziness, nasal congestion, or a bigger drop in blood pressure, especially when you stand up.

That said, I want to keep this in perspective. This concern comes only from lab dish studies, and a small human study actually found oral THC did not block CYP2C9. So a real-world problem is unlikely. Still, since low blood pressure is the main worry, mention any THC use to your pharmacist or doctor.

Macitentan, Tadalafil also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, THC may increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that cannabis containing THC might modestly inhibit the activity of CYP3A4 enzymes, which might decrease the metabolism of CYP3A4 substrates. It is unclear if this effect is due to THC, other constituents, or the combination. In vitro research also shows that cannabis extracts containing THC modestly inhibit the CYP3A4 metabolism of testosterone; extracts providing the specific cannabinoids cannabidiol (CBD) or cannabigerol (CBG) alone had stronger inhibitory effects than extracts containing both THC and CBD. Conversely, a crossover clinical study in healthy adults shows that oral THC does not inhibit CYP3A4.

Opsynvi combines two medicines for pulmonary hypertension: macitentan and tadalafil. Both are handled in part by a liver enzyme called CYP3A4. The idea here is that THC might gently slow that enzyme, which could let a bit more of each drug build up in your body. If that happened, you might notice more of their usual effects, like low blood pressure, dizziness, headache, flushing, or leg swelling.

Here's the reassuring part: this is a theoretical, minor concern based only on lab studies, and one human study actually showed oral THC did not block CYP3A4. So a real-world problem is unlikely. Still, since blood pressure changes matter, it's worth mentioning your cannabis use to your pharmacist or doctor, and don't change anything on your own.
