Why this applies to Naloxegol

Naloxegol falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Delta-9-tetrahydrocannabinol (thc) interacts with.

Theoretically, THC may increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that cannabis containing THC might modestly inhibit the activity of CYP3A4 enzymes, which might decrease the metabolism of CYP3A4 substrates. It is unclear if this effect is due to THC, other constituents, or the combination. In vitro research also shows that cannabis extracts containing THC modestly inhibit the CYP3A4 metabolism of testosterone; extracts providing the specific cannabinoids cannabidiol (CBD) or cannabigerol (CBG) alone had stronger inhibitory effects than extracts containing both THC and CBD. Conversely, a crossover clinical study in healthy adults shows that oral THC does not inhibit CYP3A4.

Here's the gist with Movantik (naloxegol) and THC. Naloxegol is cleared by a liver enzyme called CYP3A4, and lab studies suggest THC might slow that enzyme down a touch. In theory, that could let a little more naloxegol build up, which might mean stronger tummy effects like cramping, diarrhea, or belly pain. Naloxegol is not a prodrug, so it works as-is once you swallow it.

The good news: this is a very weak, theoretical concern. It comes from test-tube data, and one human study actually showed oral THC didn't block CYP3A4 at all. So real-world risk is low. Still, if you notice new gut upset after combining them, give your pharmacist or doctor a quick call.

What Is Naloxegol Used For?

Brand name: Movantik

Movantik (naloxegol) is used to treat opioid-induced constipation (OIC) — constipation caused by opioid pain medications — in adults who have chronic non-cancer pain. It is also approved for adults with chronic pain related to prior cancer or its treatment, as long as they don't need frequent (roughly weekly) increases in their opioid dose.

It is not indicated for constipation from other causes, and it is not approved for use in children.
