Here's the gist for Movantik (naloxegol). Your liver clears this drug using an enzyme called CYP3A4, and St. John's wort revs that enzyme up. When the enzyme works faster, your body breaks down naloxegol more quickly, so blood levels drop. That can leave you with less relief and your opioid-related constipation may simply come back.

This is a real, fairly well-documented effect, not just a lab guess, especially with St. John's wort products that contain meaningful amounts of hyperforin. Naloxegol's own labeling actually warns against pairing it with strong enzyme inducers like this one. So before combining them, please check with your pharmacist or doctor so they can sort out a safer plan.

What Is Naloxegol Used For?

Brand name: Movantik

Movantik (naloxegol) is used to treat opioid-induced constipation (OIC) — constipation caused by opioid pain medications — in adults who have chronic non-cancer pain. It is also approved for adults with chronic pain related to prior cancer or its treatment, as long as they don't need frequent (roughly weekly) increases in their opioid dose.

It is not indicated for constipation from other causes, and it is not approved for use in children.
