Pomalidomide falls into 2 of those categories, so it can interact with Delta-9-tetrahydrocannabinol (thc) in 2 different ways. P-GLYCOPROTEIN SUBSTRATES
1 of 2Why this applies to Pomalidomide
Pomalidomide falls under P-GLYCOPROTEIN SUBSTRATES — the group Delta-9-tetrahydrocannabinol (thc) interacts with.
Most in vitro research suggests that THC can inhibit P-gp and increase the accumulation of probe compounds by reducing P-gp mediated drug efflux. In vitro studies in kidney cell lines show that a 1-hour exposure to CBD and THC inhibits P-gp. THC may also alter the expression of P-gp, although this effect appears to vary based on duration of exposure. Some in vitro research in lymphoblastoid leukemia cell lines indicates that a 1-hour exposure to cannabinoids does not affect P-gp expression, while a prolonged 72-hour exposure decreases P-gp expression. Other in vitro research in these cell lines shows that a 4-hour exposure to THC and CBD induces P-gp gene expression, while exposure for longer than 4 hours and up to 48 hours does not induce P-gp gene expression.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for Pomalyst (pomalidomide): it's carried in and out of your cells partly by a pump called P-glycoprotein (P-gp). Lab studies suggest THC may slow that pump down. If that happened in real life, less pomalidomide would get pumped out, so a bit more could build up in your body. Pomalidomide isn't a prodrug, so more drug simply means a stronger effect and possibly more side effects, like low blood counts, fatigue, infection risk, or blood clots.
That said, this is theoretical and based only on cell and animal studies, not people. I'm not raising an alarm, but because pomalidomide is a serious cancer medicine, please loop in your oncologist or pharmacist before combining it with any THC product.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2Pomalidomide also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
In vitro research shows that cannabis containing THC might modestly inhibit the activity of CYP3A4 enzymes, which might decrease the metabolism of CYP3A4 substrates. It is unclear if this effect is due to THC, other constituents, or the combination. In vitro research also shows that cannabis extracts containing THC modestly inhibit the CYP3A4 metabolism of testosterone; extracts providing the specific cannabinoids cannabidiol (CBD) or cannabigerol (CBG) alone had stronger inhibitory effects than extracts containing both THC and CBD. Conversely, a crossover clinical study in healthy adults shows that oral THC does not inhibit CYP3A4.

Here's the gist: your body uses an enzyme called CYP3A4 to help clear pomalidomide out of your system. Some lab studies suggest THC might slightly slow that enzyme down. In theory, that could let pomalidomide build up a bit, which might mean more of its usual side effects, things like low blood counts, fatigue, increased infection risk, or blood clots.
That said, I want to keep this in perspective. This is rated as a minor and unlikely interaction, based only on test-tube research. In fact, a small human study found oral THC did not block CYP3A4 at all. So the real-world risk is probably small. Since pomalidomide is a serious cancer medicine, please check with your oncologist or pharmacist before adding THC in any form.

What Is Pomalidomide Used For?
Brand name: Pomalyst
Pomalidomide capsules are used in adults with multiple myeloma (a blood cancer affecting plasma cells) who have already tried at least two prior treatments — including lenalidomide and a proteasome inhibitor — and whose disease has continued to progress. It is given in combination with dexamethasone for this use.
Pomalidomide capsules are also used to treat Kaposi sarcoma (KS) — a cancer that can cause lesions on the skin and in internal organs — both in adults with AIDS-related KS after antiretroviral therapy has stopped working, and in adults with KS who are HIV-negative. The KS indication was approved based on response rate; ongoing studies are confirming long-term benefit.
