Here's the thing to know about combining cannabis with tipranavir (Aptivus). Tipranavir, especially boosted with ritonavir, blocks an enzyme in your body called CYP3A4. That same enzyme helps break down THC. So when you slow that enzyme down, THC can hang around longer and build up, which may make its effects stronger than you expect, things like feeling more high, dizzy, drowsy, anxious, or having a racing heart.
I'll be honest, this is a theoretical concern based on how these substances are processed, not something proven in studies, so don't panic. But if you use cannabis in any form, please mention it to your pharmacist or doctor so they can keep an eye on you and adjust if needed.

P-GLYCOPROTEIN SUBSTRATES

Most in vitro research suggests that THC can inhibit P-gp and increase the accumulation of probe compounds by reducing P-gp mediated drug efflux. In vitro studies in kidney cell lines show that a 1-hour exposure to CBD and THC inhibits P-gp. THC may also alter the expression of P-gp, although this effect appears to vary based on duration of exposure. Some in vitro research in lymphoblastoid leukemia cell lines indicates that a 1-hour exposure to cannabinoids does not affect P-gp expression, while a prolonged 72-hour exposure decreases P-gp expression. Other in vitro research in these cell lines shows that a 4-hour exposure to THC and CBD induces P-gp gene expression, while exposure for longer than 4 hours and up to 48 hours does not induce P-gp gene expression.

Here's the gist for tipranavir (Aptivus): it's moved around the body partly by a transporter called P-glycoprotein (P-gp), which acts like a bouncer pushing the drug back out of cells. Lab studies suggest THC may slow that bouncer down. In theory, that could let a bit more tipranavir build up, which might mean more of its usual side effects, like nausea, diarrhea, stomach upset, or tiredness.
That said, I want to keep this in perspective. This concern comes only from test-tube and cell studies, not from people, so we really don't know if it matters in real life. Since tipranavir is a key part of your HIV treatment, please don't change anything on your own. Just have a quick chat with your pharmacist or HIV provider before mixing in THC.

In vitro research shows that THC moderately inhibits the CYP2C9-mediated 7-hydroxylation of S-warfarin in a concentration-dependent manner. In vitro research also shows that cannabis extracts containing THC modestly inhibit the CYP2C9 metabolism of tolbutamide; extracts providing the specific cannabinoids cannabidiol (CBD) or cannabigerol (CBG) alone had stronger inhibitory effects than extracts containing both THC and CBD. Theoretically, THC may inhibit the metabolism of other CYP2C9 substrates. Conversely, a crossover clinical study in healthy adults shows that oral THC does not inhibit CYP2C9.

Here's the gist for tipranavir (Aptivus). The concern on file is that THC might slow down an enzyme called CYP2C9, which helps your body process certain drugs. If that enzyme is blocked, a drug it handles can build up a bit and cause more side effects. For tipranavir, that could in theory mean more stomach upset, nausea, or liver-related effects.
That said, I wouldn't lose sleep over this one. The evidence is very weak, coming only from lab and test-tube studies, and one actual human study found that oral THC did not block CYP2C9. Tipranavir is also given with ritonavir, which is the heavy hitter driving its blood levels. Still, since HIV therapy matters a lot, talk with your pharmacist or doctor before adding any THC product.

Here's the basic idea with tipranavir (Aptivus) and THC. Your body clears tipranavir partly through a liver enzyme called CYP3A4. Some lab-dish studies suggest THC might modestly slow that enzyme down. If that happened in real life, tipranavir levels could creep up a little, which in theory might mean more of its usual side effects, things like nausea, diarrhea, stomach upset, or tiredness.
The good news is this is mostly theory. The evidence comes from test-tube work, and a small human study actually showed oral THC did not block CYP3A4. So a real-world problem is considered unlikely. Keep in mind tipranavir is always taken boosted with ritonavir, which already strongly affects this enzyme. Still, since you're on HIV therapy, it's worth a quick word with your pharmacist or doctor before adding THC.

What Is Tipranavir Used For?
Brand name: Aptivus
Aptivus (tipranavir) is used to treat HIV-1 infection in adults and in pediatric patients weighing at least 36 kg (about 79 lbs). It is specifically for people who have already tried other HIV treatments and whose virus has become resistant to more than one protease inhibitor. It is always taken together with a low dose of ritonavir and with other HIV medications.
Aptivus is not recommended for people who have never taken HIV treatment before (treatment-naïve patients). Using tipranavir alongside additional active HIV drugs improves the chances of it working well.
