Here's the situation if you're using THC (cannabis or dronabinol) while taking Tukysa (tucatinib) for your breast or colorectal cancer. Tucatinib blocks a liver enzyme called CYP3A4, and that same enzyme helps your body clear THC. So in theory, tucatinib could slow how fast THC breaks down, letting it build up a bit. That might make THC's effects stronger than you expect, things like feeling more high, dizzy, drowsy, foggy-headed, or having a faster heartbeat.

I want to be honest: this concern is theoretical, based on how these substances work on paper rather than real patient reports, so it may not amount to much. Still, since you're going through cancer treatment, please loop in your oncologist or pharmacist before mixing the two so they can keep an eye on you.

P-GLYCOPROTEIN SUBSTRATES

Tucatinib also falls under P-GLYCOPROTEIN SUBSTRATES.

Theoretically, THC might alter levels of drugs that are substrates of P-glycoprotein (P-gp).
Most in vitro research suggests that THC can inhibit P-gp and increase the accumulation of probe compounds by reducing P-gp mediated drug efflux. In vitro studies in kidney cell lines show that a 1-hour exposure to CBD and THC inhibits P-gp. THC may also alter the expression of P-gp, although this effect appears to vary based on duration of exposure. Some in vitro research in lymphoblastoid leukemia cell lines indicates that a 1-hour exposure to cannabinoids does not affect P-gp expression, while a prolonged 72-hour exposure decreases P-gp expression. Other in vitro research in these cell lines shows that a 4-hour exposure to THC and CBD induces P-gp gene expression, while exposure for longer than 4 hours and up to 48 hours does not induce P-gp gene expression.

Here's the gist with Tucatinib (Tukysa). THC, in lab dish studies, seems to slow down a cellular pump called P-glycoprotein (P-gp). Tucatinib is moved around by that same pump, so the theory is that THC could let a little more of it build up in your body. Tucatinib is an active drug, not a prodrug, so more of it means a stronger effect and potentially more side effects, things like diarrhea, nausea, hand-foot skin reactions, fatigue, or shifts in your liver numbers.

I want to be honest about the strength here: this is based only on test-tube and animal data, not real patients, so we don't truly know if it matters in everyday life. Still, since Tucatinib is cancer treatment, please loop in your oncology team or pharmacist before mixing the two.

Tucatinib also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, THC may increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that cannabis containing THC might modestly inhibit the activity of CYP3A4 enzymes, which might decrease the metabolism of CYP3A4 substrates. It is unclear if this effect is due to THC, other constituents, or the combination. In vitro research also shows that cannabis extracts containing THC modestly inhibit the CYP3A4 metabolism of testosterone; extracts providing the specific cannabinoids cannabidiol (CBD) or cannabigerol (CBG) alone had stronger inhibitory effects than extracts containing both THC and CBD. Conversely, a crossover clinical study in healthy adults shows that oral THC does not inhibit CYP3A4.

Here's the gist for Tucatinib (Tukysa) with THC. Your body clears tucatinib using a liver enzyme called CYP3A4. In the test tube, THC-containing cannabis appears to mildly slow that enzyme down, which could in theory let a bit more tucatinib build up in your system. Since tucatinib is an active cancer drug (not a prodrug that needs activating), higher levels could mean more of its usual side effects, things like diarrhea, nausea, hand-foot soreness, fatigue, or liver enzyme changes.

That said, this is only a theoretical, lab-based concern. One human study found oral THC did not block CYP3A4 at all, and the interaction is rated minor and unlikely. Still, with cancer therapy I'd play it safe, so please mention any cannabis use to your oncologist or pharmacist before combining.

What Is Tucatinib Used For?

Brand name: Tukysa

Tukysa (tucatinib) tablets are used in combination with trastuzumab and capecitabine to treat adults with advanced, unresectable, or metastatic HER2-positive breast cancer — including those whose cancer has spread to the brain — who have already received at least one HER2-targeted treatment.

Tukysa tablets are also used in combination with trastuzumab alone to treat adults with RAS wild-type, HER2-positive unresectable or metastatic colorectal cancer that has continued to grow after prior chemotherapy. This colorectal cancer use has received accelerated approval, meaning final approval depends on further study results confirming the benefit.
