Here's the thing about DMSO: it's known as a 'penetration enhancer,' meaning it can drag other chemicals through your skin more easily. Since penciclovir (Denavir) is a cream you dab right onto a cold sore, applying DMSO nearby could push more of the medication into your skin than usual. The science behind this comes mostly from animal work and a few small human studies, so it's a reasonable concern rather than a sure thing.

If more penciclovir gets absorbed, you might notice more local irritation where you apply it, things like extra redness, stinging, numbness, or a rash. Penciclovir isn't activated by your skin enzymes in a way DMSO would block, so this is really about absorption, not a chemical reaction. I'd skip mixing the two on the same spot and check with your pharmacist first.
