"Phentolamine falls under TOPICAL DRUGS — the group Dimethylsulfoxide (dmso) interacts with.

Topical DMSO might increase the effects and adverse effects of topical drugs.
Evidence from animal studies and limited human studies suggests that DMSO may potentiate the action of numerous topical medications by increasing their absorption through the skin. This increase in absorption seems to depend on the concentration of DMSO and the molecular weight of the medication.

Here's the simple version: DMSO is a well-known 'penetration enhancer,' meaning it helps other substances slip through the skin more easily. So if you were to apply DMSO on the same area as a topical phentolamine product, it could push more of the drug into your body than intended. Phentolamine is a blood-vessel dilator (it blocks alpha receptors), so more of it getting absorbed could show up as lightheadedness, a fast heartbeat, flushing, or a drop in blood pressure.

Phentolamine is not a prodrug, so this is a straightforward 'more absorption, stronger effect' situation. Keep in mind most phentolamine you'd encounter (like dental OraVerse or Ryzumvi eye drops) isn't a skin cream, so real overlap is uncommon. The evidence here is limited too. Still, before combining any topical DMSO with any medication, please check with your pharmacist or doctor first.
