Ranolazine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group European Barberry interacts with.

Theoretically, European barberry might increase the levels and clinical effects of CYP3A4 substrates.
There is very preliminary evidence suggesting that berberine, a constituent of European barberry, might inhibit the CYP3A4 enzyme. Theoretically, European barberry might have a similar effect.

Here's the gist for ranolazine and European barberry. Ranolazine is cleared from your body largely by a liver enzyme called CYP3A4. Berberine, the main active compound in barberry, has shown a hint in early lab work that it may slow down that enzyme. If that happens, ranolazine could build up higher than expected in your blood.

Ranolazine is one of those drugs where the dose really matters, because higher levels can affect your heart's rhythm (a change called QT prolongation) and bring on dizziness, nausea, or constipation. Right now this is only a theoretical concern based on how berberine behaves in the lab, not proven cases in people. Still, before pairing barberry with ranolazine, please check with your pharmacist or doctor first.

What Is Ranolazine Used For?

Brand names: Ranexa, Aspruzyo

Ranolazine treats chronic angina — the recurring chest pain or pressure that happens when the heart isn't getting enough blood and oxygen. It is used when other angina medicines aren't enough on their own.

Both the extended-release tablet (sold as Ranexa and generic ranolazine) and the oral powder form (sold as Aspruzyo Sprinkle) are approved for this use. Ranolazine can be combined with other heart medicines such as beta-blockers, nitrates, calcium channel blockers, anti-platelet drugs, statins, ACE inhibitors, and angiotensin receptor blockers.
