Here's the situation with Spiriva (tiotropium) and European barberry. Tiotropium is what we call an anticholinergic inhaler, meaning it works by blocking a body signal to help open up your airways in COPD. In lab studies (test tube, not people), some compounds in barberry have shown mild anticholinergic-like activity too. So the theory is that the two together might add up and produce slightly more anticholinergic effects.

If that happened, you could notice things like dry mouth, blurry vision, constipation, or trouble emptying your bladder. I want to be honest though, this is a theoretical concern based only on early lab data, not real-world reports, so for most people it's likely minor. Still, it's worth a quick mention to your pharmacist or doctor before adding a concentrated berberine supplement.
