Vorapaxar falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group European Barberry interacts with.
There is very preliminary evidence suggesting that berberine, a constituent of European barberry, might inhibit the CYP3A4 enzyme. Theoretically, European barberry might have a similar effect.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for Vorapaxar (Zontivity): your body relies on a liver enzyme called CYP3A4 to clear it out. Berberine, the main active compound in European barberry, may slow that enzyme down a bit. If it does, vorapaxar could hang around at higher levels than usual, which can mean stronger blood-thinning effects and a greater chance of bleeding, like easy bruising, nosebleeds, or bleeding that's slow to stop. That matters more here because vorapaxar is already taken alongside aspirin and Plavix.
That said, this is a theoretical concern based on how berberine behaves in lab studies, not real-world reports. Still, since extra bleeding is serious, I'd run it by your pharmacist or doctor before adding any barberry or berberine supplement.

In vitro and animal evidence suggest that berberine, a constituent of European barberry, might inhibit platelet aggregation. Theoretically, European barberry might have a similar effect.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the basic idea with Vorapaxar (Zontivity). This is already a potent blood thinner that keeps your platelets from clumping together, and it's usually taken alongside aspirin and clopidogrel. European barberry contains berberine, which in lab and animal studies seems to make platelets a little less sticky too. So the worry is simple addition: stacking barberry on top of an already strong antiplatelet drug could nudge your bleeding risk up.
To be fair, this is a theoretical concern based on test-tube and animal data, not real cases in people, so I don't want to alarm you. Still, with a drug like Vorapaxar, watch for easy bruising, nosebleeds, bleeding gums, or unusually dark stools. Please check with your pharmacist or doctor before adding barberry rather than starting or stopping anything on your own.

Zontivity (vorapaxar) is used to lower the risk of heart attack, stroke, cardiovascular death, and the need for urgent procedures to open blocked arteries. It is prescribed for people who have already had a heart attack or who have peripheral arterial disease (PAD) — a condition where arteries outside the heart become narrowed or blocked.
Zontivity is not used alone. It is given alongside aspirin and/or clopidogrel as part of a broader antiplatelet treatment plan.
