Hydrochlorothiazide, Timolol falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Feverfew interacts with.

Theoretically, feverfew might increase levels of drugs metabolized by CYP2D6.
Laboratory research shows that feverfew might inhibit CYP2D6. So far, this interaction has not been reported in humans.

This combination pill pairs hydrochlorothiazide (a water pill) with timolol, a beta-blocker. The piece that matters here is the timolol, since it's partly cleared by an enzyme called CYP2D6. In lab dishes, feverfew appears to slow that enzyme down. If that held true in people, timolol could build up a bit, which might nudge your heart rate and blood pressure lower and could leave you feeling more tired, lightheaded, or dizzy when standing.

Here's the honest part: this is only test-tube evidence and hasn't actually been seen in real patients, so I wouldn't lose sleep over it. Still, since timolol affects your heart, it's worth a quick chat with your pharmacist or doctor before adding feverfew, especially if your blood pressure already runs low.
