CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Ibrutinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Feverfew interacts with.
Laboratory research shows that feverfew might inhibit CYP3A4. So far, this interaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with these two. Your body relies on a liver enzyme called CYP3A4 to break down and clear ibrutinib. Lab research hints that feverfew may slow that same enzyme down. If that were to happen in real life, ibrutinib could build up in your blood and hit harder than intended. With this cancer medication, that could mean more bruising or bleeding, diarrhea, an irregular heartbeat, infections, or worsening fatigue.
The reassuring part: this is only a theoretical concern from test-tube studies, and it has never actually been reported in people. Still, ibrutinib is a serious drug where levels matter, so I would not start feverfew on your own. Please run it by your oncologist or pharmacist first.

Laboratory research suggests that feverfew may inhibit platelet aggregation. Additionally, in one case report, a 36-year-old patient taking feverfew 2400 mg daily for 3 months experienced vaginal bleeding and a prolonged menstrual cycle, with a modest increase in partial thromboplastin time (PTT) and prothrombin time (PT).

Here's the thing to know about ibrutinib (Imbruvica): one of its known side effects is a higher risk of bleeding and bruising on its own. It can affect how your platelets work, so people on it may notice things like easy bruising, nosebleeds, or bleeding that takes longer to stop.
Feverfew, in lab studies, seems to make platelets a little less sticky too. So the worry is that stacking the two could add up and nudge your bleeding risk higher. I want to be honest with you though, this is mostly theoretical, based on lab work and one case report, not strong human studies. Still, since ibrutinib already carries real bleeding risk, it's worth a quick chat with your oncologist or pharmacist before adding feverfew.

What Is Ibrutinib Used For?
Brand name: Imbruvica
Imbruvica (ibrutinib) is used in adults to treat chronic lymphocytic leukemia (CLL) and small lymphocytic lymphoma (SLL), including cases with a specific genetic change called 17p deletion. It is also used in adults to treat Waldenström's macroglobulinemia (WM), a rare type of slow-growing blood cancer.
Imbruvica is also approved for adults and children aged 1 year and older with chronic graft-versus-host disease (cGVHD) — a serious immune complication that can occur after a stem cell or bone marrow transplant — when at least one prior treatment has not worked.
