Ibrutinib falls under Anticoagulant/Antiplatelet Drugs — the group Mullein interacts with.

Lab tests on extracts from different parts of common and dense-flowered mullein plants show that they do have some blood-thinning effects, as they increase the time it takes for blood to clot. This is likely due to at least one of the known constituents of muellein, triterpenoid saponins. However, the effects appear to be mild and results have been inconsistent. Extracts from different plant parts have been show to have varying anticoagulant strengths, with water-based ones being more potent than alcohol-based extracts.

Here's the thing to know about pairing mullein with ibrutinib (Imbruvica). Ibrutinib already carries a real bleeding risk on its own. It blocks a protein called BTK that platelets use to clump together, so people on it can bruise more easily or bleed a bit longer than usual. Mullein, in lab studies, has shown some mild blood-thinning activity, likely from plant compounds called saponins. In theory, stacking the two could nudge that bleeding risk up a little.

That said, this is gentle, inconsistent, test-tube evidence, not something proven in real patients, so I wouldn't lose sleep over a cup of mullein tea. Still, because ibrutinib's bleeding effect is well documented, it's worth a quick chat with your oncology team or pharmacist before adding mullein, especially if you notice unusual bruising or bleeding.

What Is Ibrutinib Used For?

Brand name: Imbruvica

Imbruvica (ibrutinib) is approved for adult patients with chronic lymphocytic leukemia (CLL) / small lymphocytic lymphoma (SLL), including those with a specific genetic change called 17p deletion, and for adult patients with Waldenström's macroglobulinemia (WM), a rare type of blood cancer.

Imbruvica is also approved for chronic graft-versus-host disease (cGVHD) — a complication that can occur after a stem cell transplant — in adults and children aged 1 year and older whose condition did not respond to at least one prior treatment.
