Why this applies to Bepridil

Bepridil falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Fo-ti interacts with.

Theoretically, fo-ti might increase the levels and clinical effects of drugs metabolized by CYP3A4.
In vitro research suggests that fo-ti might inhibit CYP3A4. One in vitro study suggests that the degree of CYP3A4 inhibition may depend on the type of fo-ti extract (i.e., the raw plant leads to greater inhibition than extensively processed extracts). However, this evidence conflicts with animal research suggesting that fo-ti does not inhibit CYP3A4. This interaction has not been reported in humans.

Bepridil is an older heart rhythm and chest pain medicine that your liver clears using an enzyme called CYP3A4. Lab studies hint that Fo-ti might slow this enzyme down. If that happened, your body could clear Bepridil more slowly, letting its levels creep up and possibly making side effects stronger. With Bepridil that matters more than usual, because it can affect your heart's rhythm (a risk called QT prolongation) and cause dizziness, slow heartbeat, or feeling faint.

Here's the honest part: this is only a theoretical concern from test-tube research, and it has never been reported in real people. Even so, since Bepridil is a sensitive heart drug, please check with your pharmacist or doctor before combining the two.
