Why this applies to Brentuximab Vedotin

Brentuximab Vedotin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Fo-ti interacts with.

Theoretically, fo-ti might increase the levels and clinical effects of drugs metabolized by CYP3A4.
In vitro research suggests that fo-ti might inhibit CYP3A4. One in vitro study suggests that the degree of CYP3A4 inhibition may depend on the type of fo-ti extract (i.e., the raw plant leads to greater inhibition than extensively processed extracts). However, this evidence conflicts with animal research suggesting that fo-ti does not inhibit CYP3A4. This interaction has not been reported in humans.

Here's the gist: brentuximab vedotin (Adcetris) releases a chemotherapy payload called MMAE, and your body clears that piece using an enzyme called CYP3A4. The concern with Fo-ti is that, in test-tube studies, it may slow that enzyme down. If that happened in a person, the chemo could linger longer than intended and push side effects higher, things like nerve tingling or numbness in the hands and feet, low blood counts, fatigue, nausea, and infections.

Now, the honest part: this is theoretical. It's based only on lab work, animal studies actually disagreed, and it has never been seen in real patients. Still, since this is serious chemotherapy, please don't add Fo-ti without first checking with your oncology team or pharmacist.

What Is Brentuximab Vedotin Injection Used For?

Brand name: Adcetris

Adcetris (brentuximab vedotin) is used to treat classical Hodgkin lymphoma (cHL) in adults — including newly diagnosed advanced-stage disease, as consolidation therapy after a stem cell transplant, and in relapsed or refractory cases. It is also approved for pediatric patients aged 2 and older with previously untreated high-risk classical Hodgkin lymphoma, given in combination with chemotherapy.

Adcetris is also used for several T-cell lymphomas — including systemic anaplastic large cell lymphoma (sALCL), peripheral T-cell lymphomas (PTCL), primary cutaneous anaplastic large cell lymphoma (pcALCL), and CD30-expressing mycosis fungoides (MF). Additionally, Adcetris is used in combination with lenalidomide and a rituximab product for adults with relapsed or refractory large B-cell lymphoma (LBCL) who are not candidates for a stem cell transplant or CAR T-cell therapy.
