Clozapine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Fo-ti interacts with.

Theoretically, fo-ti may increase the levels and clinical effects of drugs metabolized by CYP2D6.
Animal research suggests that fo-ti might inhibit CYP2D6. Additionally, an in vitro study suggests that the degree of CYP2D6 inhibition may depend on the type of fo-ti extract (i.e., the raw plant leads to greater inhibition than extensively processed extracts). However, this interaction has not been reported in humans.

Here's the basic idea with Clozapine and Fo-ti. Your body uses a liver enzyme called CYP2D6 to help clear clozapine. Lab and animal studies hint that Fo-ti might slow that enzyme down, especially the raw root rather than heavily processed extracts. If clozapine clears more slowly, levels could creep up, which may mean more of its known side effects like heavy drowsiness, dizziness, drooling, constipation, a fast heartbeat, or low blood pressure.

I want to be honest, though: this is theoretical and hasn't actually been reported in people, so it's more of a caution than a proven problem. Clozapine is a closely monitored medication, so don't add or stop Fo-ti on your own. Please check with your pharmacist or prescriber first.

Clozapine also falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES.

Theoretically, fo-ti might increase or decrease the levels and clinical effects of drugs metabolized by CYP1A2.
In vitro research suggests that fo-ti might inhibit CYP1A2. Additionally, in vitro research suggests that the degree of CYP1A2 inhibition depends on the type of fo-ti extract (i.e., the raw plant leads to greater inhibition than extensively processed extracts). However, in an animal study, an aqueous extract of fo-ti inhibited CYP1A2 while an alcoholic extract of fo-ti induced CYP1A2. Induction or inhibition of CYP1A2 by fo-ti has not been reported in humans.

Here's the connection worth knowing about. Your body relies on a liver enzyme called CYP1A2 to break down and clear clozapine. Lab studies hint that Fo-ti, especially the raw or less-processed root, may slow that same enzyme down. If that happened in real life, clozapine could build up higher than expected, which can mean more drowsiness, dizziness, drooling, a faster heartbeat, low blood pressure, or in rare cases seizures.

I want to be honest about the evidence though. This is only test-tube and animal data, with mixed results (some Fo-ti extracts even sped the enzyme up), and it has never been shown in people. Still, clozapine is a sensitive, closely-monitored medicine, so don't add Fo-ti without first checking with your prescriber or pharmacist.

What Is Clozapine Used For?

Clozapine is used for two specific situations. First, it is prescribed for people with severe schizophrenia who have already tried other antipsychotic medications and have not gotten adequate relief. It is reserved for this group because of its serious risks.

Second, clozapine is used to lower the chances of repeated suicidal behavior in people with schizophrenia or schizoaffective disorder who are considered to be at ongoing, chronic risk. It is not a first-choice medication — it is used when the need is serious and other options have fallen short.
