Drospirenone, Estetrol falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Fo-ti interacts with.

Theoretically, fo-ti might increase or decrease the levels and clinical effects of drugs metabolized by CYP1A2.
In vitro research suggests that fo-ti might inhibit CYP1A2. Additionally, in vitro research suggests that the degree of CYP1A2 inhibition depends on the type of fo-ti extract (i.e., the raw plant leads to greater inhibition than extensively processed extracts). However, in an animal study, an aqueous extract of fo-ti inhibited CYP1A2 while an alcoholic extract of fo-ti induced CYP1A2. Induction or inhibition of CYP1A2 by fo-ti has not been reported in humans.

Here's the honest take on combining Fo-ti with Nextstellis (your drospirenone and estetrol birth control pill). The concern on file is that Fo-ti may affect a liver enzyme called CYP1A2. The thing is, this enzyme isn't a major player in how your body handles these particular hormones, and the whole concern is theoretical, coming only from lab and animal studies. Nobody has actually seen this happen in real people, and the effect could go either way depending on the type of Fo-ti.

So I wouldn't lose sleep over this specific enzyme issue. My bigger flag with Fo-ti is that processed forms have been linked to liver injury, and your pill is processed by the liver too. Please loop in your pharmacist or doctor before pairing them.

Here's the concern with combining Fo-ti and your birth control pill, Nextstellis (which contains drospirenone and estetrol). Both of these can put a little extra work on your liver. Fo-ti, especially the processed root, has shown up in a number of case reports linked to liver injury, and estrogen-containing birth control is also processed by the liver. Stacking two things that can stress the liver may raise that risk.

To be clear, this is mostly a theoretical and anecdotal concern, not something proven to happen often. Still, I'd keep an eye out for signs like yellowing of the skin or eyes, dark urine, belly pain, unusual tiredness, or nausea. Please don't stop either one on your own. Just check with your pharmacist or doctor before pairing them.

Nextstellis pairs drospirenone with estetrol, a plant-like estrogen, to prevent pregnancy. The concern here is theoretical: in test-tube studies, fo-ti showed some weak estrogen-like activity, and in large supplement doses it could in theory compete at estrogen receptors and nudge how reliably your birth control works. This is not proven in people, so I don't want to overstate it.

Since this is one of the things you really don't want guessing on, I'd keep it simple. If you take fo-ti regularly, mention it to your pharmacist or doctor, and don't rely on it changing anything. Also worth knowing: fo-ti has been linked to liver injury in some reports, which is its own reason to be cautious.

Nextstellis combines estetrol (a plant-like estrogen) with drospirenone, and it's used for birth control. The concern here is that fo-ti has shown estrogen-like activity in lab dish studies, meaning some of its compounds may latch onto the same estrogen receptors your pill works on. In theory, large amounts of concentrated fo-ti could compete with or nudge the hormonal balance this pill is trying to create.

I want to be honest with you: this is based only on test-tube research, not on real people, so we don't truly know if it matters in everyday life. Still, since reliable birth control is the goal, and processed fo-ti has also been tied to liver concerns, it's worth a quick chat with your pharmacist or doctor before combining them.

However, this evidence conflicts with animal research suggesting that fo-ti does not inhibit CYP3A4. This interaction has not been reported in humans.

Nextstellis combines two active hormones, drospirenone and estetrol, and both rely partly on the CYP3A4 enzyme to be broken down in your body. The thinking here is that Fo-ti might slow that enzyme down a bit. If it did, more of the hormone could hang around, which in theory could nudge up estrogen-type side effects like nausea, breast tenderness, headaches, or bloating.

Honestly though, this is a very soft signal. It comes only from lab dish studies, animal research actually disagreed, and it has never been reported in a real person. So I wouldn't lose sleep over it. Just keep your pharmacist or doctor in the loop before adding Fo-ti, especially since processed Fo-ti has been tied to liver concerns.
