Why this applies to Lapatinib

Lapatinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Fo-ti interacts with.

Theoretically, fo-ti might increase the levels and clinical effects of drugs metabolized by CYP3A4.
In vitro research suggests that fo-ti might inhibit CYP3A4. One in vitro study suggests that the degree of CYP3A4 inhibition may depend on the type of fo-ti extract (i.e., the raw plant leads to greater inhibition than extensively processed extracts). However, this evidence conflicts with animal research suggesting that fo-ti does not inhibit CYP3A4. This interaction has not been reported in humans.

Here's the situation with Fo-ti and lapatinib (Tykerb). Your body clears lapatinib mainly through a liver enzyme called CYP3A4. Lab studies hint that Fo-ti, especially the raw root, may slow that same enzyme down. If that happened in people, lapatinib could build up to higher levels than intended, which can mean more of its typical side effects: diarrhea, nausea, mouth sores, rash, hand-foot skin reactions, and in some cases effects on the liver or heart rhythm.

I want to be honest about the strength here. This is theoretical, based only on test-tube and animal work, and it hasn't been seen in actual patients. Animal data even disagree. Still, since lapatinib is serious cancer therapy, and Fo-ti itself has been tied to liver injury, please loop in your oncologist or pharmacist before combining them.

What Is Lapatinib Used For?

Brand name: Tykerb

Lapatinib tablets — sold under the brand name Tykerb — are used to treat advanced or metastatic breast cancer in patients whose tumors overexpress the HER2 protein. They are always used in combination with another medicine, not alone.

Tykerb is approved in combination with capecitabine for HER2-positive advanced or metastatic breast cancer in patients who have already received prior treatment including an anthracycline, a taxane, and trastuzumab, and whose disease has progressed on trastuzumab. Tykerb is also approved in combination with letrozole for postmenopausal women with hormone receptor-positive, HER2-positive metastatic breast cancer when hormonal therapy is appropriate.
