Here's the concern with taking Fo-ti alongside Opsynvi (which combines macitentan and tadalafil). This isn't about Fo-ti speeding up or slowing down how your body clears the drug. Instead, it's about the liver itself. Fo-ti has been tied to liver injury in a number of case reports, and macitentan is already a medicine we watch closely for liver strain. Doctors often check liver blood tests while you're on it.

Stacking two things that can each stress the liver may raise your risk of problems like fatigue, nausea, belly pain, dark urine, or yellowing of the skin or eyes. To be fair, this warning is theoretical and based mostly on scattered reports, not strong studies. Still, given macitentan's known liver cautions, I'd talk with your doctor or pharmacist before combining them, and don't stop anything on your own.

Theoretically, fo-ti may increase the levels and clinical effects of drugs metabolized by CYP2C9.
Animal and in vitro research suggests that fo-ti may inhibit CYP2C9. However, this interaction has not been reported in humans.

Opsynvi combines macitentan (for pulmonary hypertension) with tadalafil (the same medicine as Cialis). Both are handled in part by a liver enzyme called CYP2C9. Lab and animal studies hint that Fo-ti might slow this enzyme down. If that happened in people, these two drugs could build up a bit more than expected.

What could that look like? Mostly the known effects of these drugs getting stronger: lightheadedness, a bigger drop in blood pressure, headache, flushing, or nasal stuffiness. Here's the honest part though: this is only a theoretical concern from test-tube work, never actually seen in humans. Fo-ti also carries its own liver-injury reports, so I'd loop in your doctor or pharmacist before combining them.

Theoretically, fo-ti might increase the levels and clinical effects of drugs metabolized by CYP3A4.
In vitro research suggests that fo-ti might inhibit CYP3A4. One in vitro study suggests that the degree of CYP3A4 inhibition may depend on the type of fo-ti extract (i.e., the raw plant leads to greater inhibition than extensively processed extracts). However, this evidence conflicts with animal research suggesting that fo-ti does not inhibit CYP3A4. This interaction has not been reported in humans.

Opsynvi combines two medicines, macitentan and tadalafil, and both are cleared by a liver enzyme called CYP3A4. Here's the concern with Fo-ti: some lab studies suggest it may slow that enzyme down. If that happened in a person, these drugs could build up a bit more than expected. In plainer terms, you might see more of tadalafil's effects, like headache, flushing, stomach upset, or lightheadedness when you stand, since both drugs can lower blood pressure.

Now, the honest part: this is only test-tube data, animal studies actually disagree, and it has never been reported in a real patient. So I wouldn't lose sleep over it, but because Opsynvi treats pulmonary hypertension, please loop in your doctor or pharmacist before adding Fo-ti.
