Why this applies to Naproxen, Sumatriptan

Naproxen, Sumatriptan falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Fo-ti interacts with.

Theoretically, fo-ti might increase or decrease the levels and clinical effects of drugs metabolized by CYP1A2.
In vitro research suggests that fo-ti might inhibit CYP1A2. Additionally, in vitro research suggests that the degree of CYP1A2 inhibition depends on the type of fo-ti extract (i.e., the raw plant leads to greater inhibition than extensively processed extracts). However, in an animal study, an aqueous extract of fo-ti inhibited CYP1A2 while an alcoholic extract of fo-ti induced CYP1A2. Induction or inhibition of CYP1A2 by fo-ti has not been reported in humans.

Treximet combines two active medicines, naproxen (an anti-inflammatory pain reliever) and sumatriptan (a migraine medication). Neither one is a prodrug, so they work as-is without needing to be switched on. The concern here is that fo-ti may slow down a liver enzyme called CYP1A2, which plays a part in clearing certain drugs. If that happens, these medicines could linger a bit longer and feel stronger.

Honestly though, this is a theoretical worry based only on lab and animal studies, and it has never been seen in real people. The findings were even mixed, with some fo-ti extracts speeding the enzyme up and others slowing it down. If anything did shift, you might notice more naproxen side effects like stomach upset, or extra drowsiness or chest tightness from the sumatriptan. I wouldn't lose sleep over it, but since fo-ti has also been tied to liver issues, please run it by your pharmacist or doctor first.

Theoretically, fo-ti may increase the levels and clinical effects of drugs metabolized by CYP2C9.
Animal and in vitro research suggests that fo-ti may inhibit CYP2C9. However, this interaction has not been reported in humans.

Treximet combines two medicines, and one of them, naproxen, is partly cleared by a liver enzyme called CYP2C9. Lab and animal studies hint that Fo-ti may slow this enzyme down. If that held true in people, naproxen could build up a bit and linger longer, which might mean more of its usual side effects, like stomach upset, heartburn, an increased risk of stomach irritation or bleeding, or fluid retention.

Here's the reassuring part: this is only a theoretical concern. It has not been seen in actual humans, just in test tubes and animals. Naproxen is not a prodrug, so the worry is more drug, not less. Still, if you take Treximet regularly, it's worth a quick chat with your pharmacist or doctor before adding Fo-ti.

ANTICOAGULANT/ANTIPLATELET DRUGS

Naproxen, Sumatriptan also falls under ANTICOAGULANT/ANTIPLATELET DRUGS.

Fo-ti has been linked to cases of acute liver failure which can decrease clotting factor production and increase the effects of anticoagulants. In one case, a patient who had been stable on warfarin presented with acute hepatitis and an INR elevated to 14.98. The patient had been taking fo-ti for 90 days prior to admission. Discontinuation of warfarin and fo-ti lead to a decrease in the INR and full recovery. Theoretically, concomitant use of fo-ti with anticoagulant or antiplatelet drugs may increase the risk of bleeding in some patients. Until more is known, monitor patients taking fo-ti and drugs that affect bleeding.
Some of these drugs include aspirin, clopidogrel (Plavix), dalteparin (Fragmin), dipyridamole (Persantine), enoxaparin (Lovenox), heparin, ticlopidine (Ticlid), warfarin (Coumadin), and others.

Treximet combines naproxen (an NSAID) with sumatriptan (a migraine medicine). The piece that matters here is naproxen, because NSAIDs like it can already irritate the stomach lining and make it a little harder for your platelets to do their clotting job. On its own, that's usually a manageable risk.

The concern with Fo-ti is more indirect. It has been tied to rare cases of liver injury, and a struggling liver makes fewer clotting factors. Pair that with naproxen's effect on platelets and you could, in theory, bruise more easily, have a nosebleed that won't quit, or notice black or bloody stools. This is based on anecdotal reports, not strong studies, so don't panic. Still, please run this combo by your pharmacist or doctor before mixing them.
