Why this applies to Osilodrostat Phosphate

Osilodrostat Phosphate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Fo-ti interacts with.

Theoretically, fo-ti might increase the levels and clinical effects of drugs metabolized by CYP3A4.
In vitro research suggests that fo-ti might inhibit CYP3A4. One in vitro study suggests that the degree of CYP3A4 inhibition may depend on the type of fo-ti extract (i.e., the raw plant leads to greater inhibition than extensively processed extracts). However, this evidence conflicts with animal research suggesting that fo-ti does not inhibit CYP3A4. This interaction has not been reported in humans.

Here's the gist with Isturisa (osilodrostat) and Fo-ti. Your liver uses an enzyme called CYP3A4 to help clear osilodrostat from your body. In test-tube studies, Fo-ti seemed to slow that same enzyme down. If that happened in a real person, osilodrostat could build up a bit higher than expected, which might mean stronger effects and more side effects like nausea, tiredness, headache, swelling, low potassium, or a longer heartbeat interval on an EKG.

That said, I want to be honest about the evidence. This is only lab-dish data, animal studies didn't agree, and it has never actually been reported in people. So it's more of a theoretical caution than a proven problem. Still, please run it by your pharmacist or doctor before combining them.

Here's the gist with Isturisa (osilodrostat) and Fo-ti. Osilodrostat is partly broken down by a liver enzyme called CYP2B6. Lab and animal studies hint that Fo-ti might slow that enzyme down, which in theory could let osilodrostat build up a bit more than expected. Since osilodrostat is an active drug (not a prodrug), more of it hanging around could mean stronger effects, like a sharper drop in cortisol, low potassium, or QT-related heart rhythm changes that this medicine is already watched for.

Now the reassuring part: this is only a theoretical, lab-based concern and has never actually been reported in people. Still, since osilodrostat needs careful monitoring, I'd mention any Fo-ti use to your doctor or pharmacist before combining them.
