Tamoxifen Citrate falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Fo-ti interacts with.
Animal research suggests that fo-ti might inhibit CYP2D6. Additionally, an in vitro study suggests that the degree of CYP2D6 inhibition may depend on the type of fo-ti extract (i.e., the raw plant leads to greater inhibition than extensively processed extracts). However, this interaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's a wrinkle worth knowing: tamoxifen is actually a prodrug. Your body uses an enzyme called CYP2D6 to convert it into endoxifen, the form that does most of the real work against breast cancer. So with tamoxifen, anything that blocks CYP2D6 could mean less of the active drug, not more.
Lab and animal studies hint that fo-ti (especially the raw root) might slow CYP2D6 down. In theory, that could lower how much active medicine you make. I'll be honest though, this is weak evidence, it hasn't been shown in people, and tamoxifen is too important to risk guessing on. Please run any fo-ti by your oncologist or pharmacist first. Fo-ti has also been tied to liver injury, which is another reason to check.

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the concern with pairing Fo-ti and tamoxifen: it's not about changing how well your tamoxifen works, it's about your liver. Tamoxifen can occasionally stress the liver on its own, and processed Fo-ti has shown up in a number of case reports linked to liver injury. Stack two things that can both be hard on the liver, and you may raise the odds of trouble.
To be honest, this is based on scattered reports rather than strong studies, so it's a theoretical caution, not a sure thing. Still, watch for signs like yellowing skin or eyes, dark urine, belly pain, unusual tiredness, or nausea, and call your doctor if they show up. Please loop in your pharmacist or oncologist before adding Fo-ti.

Here's the thing about Fo-ti and tamoxifen. Lab and animal studies hint that Fo-ti may slow down an enzyme called CYP2C9, which is one of the pathways your body uses to handle tamoxifen. If that enzyme gets blocked, tamoxifen could build up a bit more than expected, which in theory might mean more of its usual side effects, like hot flashes, leg swelling, or mood changes.
I want to be honest though, this is only a theoretical concern. It has been seen in test tubes and animals, but never actually reported in people, so I wouldn't lose sleep over it. Since tamoxifen is such an important cancer medicine, please run any Fo-ti use by your oncologist or pharmacist first. Fo-ti has also been linked to liver problems, which is another reason to check in.

In vitro research suggests that fo-ti might inhibit CYP3A4. One in vitro study suggests that the degree of CYP3A4 inhibition may depend on the type of fo-ti extract (i.e., the raw plant leads to greater inhibition than extensively processed extracts). However, this evidence conflicts with animal research suggesting that fo-ti does not inhibit CYP3A4. This interaction has not been reported in humans.

Here's the wrinkle with tamoxifen: it's actually a prodrug. Your body has to convert it into its active form (mostly through a liver enzyme called CYP2D6, with some help from CYP3A4) before it can fight breast cancer. So the usual rule of thumb gets a little tricky here. Lab studies hint that Fo-ti, especially the raw root, might slow down CYP3A4, but the research conflicts and this has never actually been seen in real people.
Because tamoxifen is such an important medication and the evidence here is weak and theoretical, I wouldn't lose sleep, but I also wouldn't add Fo-ti on your own. Please run it by your oncologist or pharmacist first so nothing interferes with how well your treatment works.

What Is Tamoxifen Used For?
Brand names: Nolvadex, Soltamox
Tamoxifen is used to treat estrogen receptor-positive metastatic breast cancer in both women and men, and as follow-up (adjuvant) therapy after surgery for early-stage breast cancer in women. It also reduces the occurrence of cancer in the opposite breast when used as adjuvant therapy.
In women with ductal carcinoma in situ (DCIS — an early, non-invasive breast change) following surgery and radiation, tamoxifen lowers the risk of developing invasive breast cancer. It is also approved to reduce the risk of breast cancer in women who are at high risk but have not yet been diagnosed. Both Tamoxifen Citrate tablets and Soltamox oral solution share these indications.
