Why this applies to Tezacaftor, Ivacaftor

Tezacaftor, Ivacaftor falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Fo-ti interacts with.

Theoretically, fo-ti might increase the levels and clinical effects of drugs metabolized by CYP3A4.
In vitro research suggests that fo-ti might inhibit CYP3A4. One in vitro study suggests that the degree of CYP3A4 inhibition may depend on the type of fo-ti extract (i.e., the raw plant leads to greater inhibition than extensively processed extracts). However, this evidence conflicts with animal research suggesting that fo-ti does not inhibit CYP3A4. This interaction has not been reported in humans.

Both pieces of Symdeko, tezacaftor and ivacaftor, are cleared from your body mainly by a liver enzyme called CYP3A4. In test tube studies, Fo-ti appears to slow that enzyme down, especially the raw root. If that happens in real life, your body could clear Symdeko more slowly, letting levels creep up. That might mean more of the usual side effects, like headache, nausea, belly pain, dizziness, or changes in liver tests.

I want to be honest about how shaky this evidence is. It comes only from lab and animal work, the animal data actually disagree, and nothing has been seen in people. Fo-ti also carries its own liver-injury reports, which matters here. Please loop in your CF team or pharmacist before combining them.

What Is Tezacaftor and Ivacaftor Used For?

Brand name: Symdeko

SYMDEKO (tezacaftor/ivacaftor) is used to treat cystic fibrosis (CF) in patients aged 6 years and older. It is approved for people who have two copies of the F508del mutation, or at least one CFTR gene mutation that is known to respond to this combination based on lab data or clinical evidence.

Because CF is caused by many different gene mutations, your doctor may order genetic testing to confirm your mutation is one that SYMDEKO can help. Not every CFTR mutation responds to this treatment.
