Symdeko (tezacaftor and ivacaftor) treats cystic fibrosis and depends on a steady blood level, which is exactly what most of this list can upset, so it deserves genuine attention. The great majority of these ingredients act on CYP3A4, the liver enzyme that clears the drug. Some push its concentration up toward side effects, while others, St. John's wort most clearly, speed its breakdown enough to blunt treatment. Grapefruit is the everyday one to know; it raises drug levels enough to carry a major rating.

A smaller set works in the gut instead, binding or slowing pills so less medicine gets absorbed at all, with activated charcoal the strongest example and the third of the major-rated entries.
