Why this applies to Tezacaftor, Ivacaftor
Tezacaftor, Ivacaftor falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Chokeberry interacts with.
In vitro research shows that chokeberry inhibits CYP3A4. In humans, there is one case report of a drug interaction with trabectedin, a CYP3A4 substrate, which is hypothesized to have been caused by chokeberry inhibition of CYP3A4.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Both medicines in Symdeko, tezacaftor and ivacaftor, are cleared from your body mainly by a liver enzyme called CYP3A4. Chokeberry has been shown in test tube studies to slow this same enzyme down. If that happens in real life, your body could clear Symdeko a bit slower, letting levels creep up. In plain terms, you might notice more of the usual side effects, like headache, dizziness, stomach upset, diarrhea, or a rise in liver enzymes that your care team watches with blood tests.
Here's the honest part: this is mostly theoretical, based on lab work and a single case report with a different drug. Casual amounts of chokeberry juice or jam are unlikely to cause trouble. Concentrated supplements are the bigger question mark, so check with your CF team or pharmacist before adding one.

Tezacaftor and ivacaftor (Symdeko) treats cystic fibrosis (CF) in people aged 6 years and older. It is for people who have two copies of the F508del mutation. It is also for people who have at least one other CFTR gene mutation that responds to this medicine.
If your genetic type is unknown, an FDA-cleared CF mutation test is used to check for a CFTR mutation.
