Rifampin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Chokeberry interacts with.

Theoretically, chokeberry might increase levels of drugs metabolized by CYP3A4.
In vitro research shows that chokeberry inhibits CYP3A4. In humans, there is one case report of a drug interaction with trabectedin, a CYP3A4 substrate, which is hypothesized to have been caused by chokeberry inhibition of CYP3A4.

Here's the gist: chokeberry has been shown in test-tube studies to gently block an enzyme called CYP3A4, which your body uses to process many medications. The thinking is that if you blocked that enzyme, levels of those drugs could creep up. Rifampin is a little unusual here. It isn't just a passenger of CYP3A4, it's actually a powerful inducer that revs that enzyme way up, so chokeberry's mild effect is unlikely to overpower it.

Honestly, this concern rests on lab work and a single case report with a different drug, so it's very theoretical for rifampin. Food amounts of chokeberry (juice or jam) almost certainly won't matter. If you're using concentrated chokeberry supplements, just give your pharmacist or doctor a heads-up so they can keep an eye on things.

About Rifampin

What Is Rifampin Used For?

Brand names: Rifadin, Rimactane, Rifadin Injection, Rofactor

Rifampin treats all forms of tuberculosis (TB). It is almost always used in combination with other antibiotics — your doctor will build a multi-drug regimen tailored to your infection. Treatment usually runs for at least 6 months total.

Rifampin is also used to clear Neisseria meningitidis bacteria from the nose and throat in people who carry the germ without being sick themselves. It is not used to treat active meningococcal disease. The intravenous (IV) forms — Rifadin IV, Rifadin IV NOVAPLUS, and the injectable form of Rifampin — are specifically indicated when a patient cannot take the drug by mouth.
