Tezacaftor, Ivacaftor falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Grape interacts with.

It is unclear if grape seed extract inhibits or induces CYP3A4; research is conflicting.
In vitro evidence suggests that grape seed extract might inhibit CYP3A4 enzymes. However, evidence from animal research shows that grape seed extract may induce CYP3A4 in the liver. So far, these interactions have not been reported in humans.

Here's the short version: both pieces of Symdeko (tezacaftor and ivacaftor) are cleared from your body by a liver enzyme called CYP3A4. Grape seed extract may tug on that same enzyme, but honestly the research pulls in two directions. Some lab (test-tube) studies suggest it could slow the enzyme, which would let the drug build up and possibly cause more side effects like headache, nausea, dizziness, or belly upset. Other animal studies suggest the opposite, that it could speed the enzyme up and make the medicine less effective at helping your cystic fibrosis.

The key thing to know is that this has only been seen in lab and animal work, never confirmed in people, so it's theoretical for now. Everyday grapes and grape juice are very unlikely to matter. Before starting a concentrated grape seed supplement, just run it by your CF team or pharmacist.

What Is Tezacaftor and Ivacaftor Used For?

Brand name: Symdeko

SYMDEKO (tezacaftor/ivacaftor) is used to treat cystic fibrosis (CF) in patients aged 6 years and older. To qualify, a patient must either have two copies of the F508del mutation, or at least one mutation in the CFTR gene that lab data or clinical evidence shows will respond to this combination.

If a patient's genetic mutation is not already known, an FDA-cleared CF genetic test is recommended to confirm whether they have a mutation that responds to SYMDEKO.
