Tezacaftor, Ivacaftor falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ayahuasca interacts with.

Theoretically, ayahuasca might increase plasma concentrations of CYP3A4 substrates.
Ayahuasca contains beta-carboline alkaloids. In vitro research suggests that beta-carboline alkaloids competitively inhibit CYP3A4 enzymes. This effect has not been reported in humans.

Here's the gist for your Symdeko (tezacaftor/ivacaftor). Both of these are cleared from your body mainly by a liver enzyme called CYP3A4. Ayahuasca contains beta-carboline compounds (like harmine) that, in lab studies, can slow that same enzyme down. If that happened in a real person, your Symdeko could build up higher than expected, which might mean more side effects such as headache, dizziness, nausea, stomach upset, or changes in liver enzyme levels.

I want to be honest about the strength here: this is theoretical, based only on test-tube research, and hasn't been shown in people. That said, ayahuasca is a potent, risky brew on its own. Please don't combine these without first talking to your CF team or pharmacist.

What Is Tezacaftor and Ivacaftor Used For?

Brand name: Symdeko

SYMDEKO (tezacaftor/ivacaftor) is used to treat cystic fibrosis (CF) in patients aged 6 years and older. It is approved for people who have two copies of the F508del mutation, or at least one CFTR gene mutation that is known to respond to this combination based on lab data or clinical evidence.

Because CF is caused by many different gene mutations, your doctor may order genetic testing to confirm your mutation is one that SYMDEKO can help. Not every CFTR mutation responds to this treatment.
