Tezacaftor, Ivacaftor falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Sulforaphane interacts with.

Theoretically, sulforaphane might increase the levels and effects of CYP3A4 substrates.
Animal and in vitro research shows that sulforaphane inhibits CYP3A4 enzyme activity and downregulates its expression. This interaction has not been reported in humans.

Here's the gist for your Symdeko (tezacaftor/ivacaftor). Both of these active medications are cleared by a liver enzyme called CYP3A4. In lab and animal studies, sulforaphane appears to slow that enzyme down. If it does the same in people, your body would break down the medication more slowly, so levels could creep up. That might mean a higher chance of side effects like headache, nausea, stomach pain, diarrhea, or dizziness.

The big thing to keep in mind: this is only theoretical right now, based on test-tube and animal work, and it has never actually been reported in humans. Eating broccoli and other cruciferous veggies is not a concern. A concentrated sulforaphane supplement is the only real question, so just run it by your CF team or pharmacist before starting one.

Brand name: Symdeko

SYMDEKO (tezacaftor/ivacaftor) is used to treat cystic fibrosis (CF) in patients aged 6 years and older. It is approved for people who have two copies of the F508del mutation, or at least one CFTR gene mutation that is known to respond to this combination based on lab data or clinical evidence.

Because CF is caused by many different gene mutations, your doctor may order genetic testing to confirm your mutation is one that SYMDEKO can help. Not every CFTR mutation responds to this treatment.
