Why this applies to Doxorubicin

Doxorubicin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Sulforaphane interacts with.

Theoretically, sulforaphane might increase the levels and effects of CYP3A4 substrates.
Animal and in vitro research shows that sulforaphane inhibits CYP3A4 enzyme activity and downregulates its expression. This interaction has not been reported in humans.

Here's the basic idea with this pairing. Doxorubicin is partly broken down by a liver enzyme called CYP3A4. In lab dishes and animal studies, sulforaphane (the active compound concentrated from broccoli) seems to slow that enzyme down. If that happened in people, your body might clear doxorubicin a little more slowly, letting more of it linger. With this chemo drug, more exposure could mean stronger side effects like nausea, mouth sores, low blood counts, and strain on the heart.

The honest part: this is theoretical and has never been confirmed in humans, so don't panic. Food amounts of broccoli are very unlikely to matter. Concentrated supplements during chemo are a different story, so please run any sulforaphane product by your oncology team or pharmacist first.

Doxorubicin is used to treat a wide range of cancers. It is given as part of combination chemotherapy for women with breast cancer that has spread to the lymph nodes after surgery, and it is used for many other cancers — from blood cancers like leukemia and lymphoma to solid tumors like bone and soft tissue sarcomas.

It is considered a broad-spectrum cancer fighter, meaning it is active against many different tumor types. It is almost always given alongside other chemotherapy drugs rather than on its own.
