Why this applies to Rifampin

Rifampin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Sulforaphane interacts with.

Theoretically, sulforaphane might increase the levels and effects of CYP3A4 substrates.
Animal and in vitro research shows that sulforaphane inhibits CYP3A4 enzyme activity and downregulates its expression. This interaction has not been reported in humans.

Here's the idea behind this one. Lab and animal studies hint that sulforaphane may slow down a liver enzyme called CYP3A4. Since rifampin is partly handled by that enzyme, blocking it could, in theory, let a little more rifampin linger in your system. That might mean a slightly higher chance of side effects like stomach upset, nausea, or temporary changes in liver tests. (Rifampin itself usually turns this enzyme up, so the real-world push and pull here is honestly hard to predict.)

The important part: this has only been seen in test tubes and animals, never confirmed in people, so I wouldn't lose sleep over broccoli on your plate. Concentrated capsules are a different story. Since rifampin is treating something serious like TB, please loop in your pharmacist or doctor before adding a supplement.

About Rifampin

What Is Rifampin Used For?

Brand names: Rifadin, Rimactane, Rifadin Injection, Rofactor

Rifampin treats all forms of tuberculosis (TB). It is almost always used in combination with other antibiotics — your doctor will build a multi-drug regimen tailored to your infection. Treatment usually runs for at least 6 months total.

Rifampin is also used to clear Neisseria meningitidis bacteria from the nose and throat in people who carry the germ without being sick themselves. It is not used to treat active meningococcal disease. The intravenous (IV) forms — Rifadin IV, Rifadin IV NOVAPLUS, and the injectable form of Rifampin — are specifically indicated when a patient cannot take the drug by mouth.
