Why this applies to Nifedipine

Nifedipine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Sulforaphane interacts with.

Theoretically, sulforaphane might increase the levels and effects of CYP3A4 substrates.
Animal and in vitro research shows that sulforaphane inhibits CYP3A4 enzyme activity and downregulates its expression. This interaction has not been reported in humans.

Here's the quick version. Your body relies on a liver enzyme called CYP3A4 to break down nifedipine. In lab dishes and animal studies, sulforaphane (the broccoli compound) appears to slow that enzyme down. If that same effect happened in people, nifedipine wouldn't clear as quickly, so more of it could linger in your bloodstream and its blood-pressure-lowering action could get stronger than expected.

What might that feel like? Possibly more dizziness, flushing, headache, ankle swelling, or a faster heartbeat, especially when standing up. That said, this is only a theoretical concern from test-tube and animal data, and it hasn't actually been seen in humans. Eating broccoli is very unlikely to matter. A concentrated sulforaphane supplement is worth running past your pharmacist or doctor first.

What Is Nifedipine Used For?

Brand names: Procardia XL, Adalat PA, Adalat, Nifedin, Nifed PA, Nifed, Procardia, Adalat XL, Adalat CC, Adalat LA, Adalat Retard, Adipine MR, Adipine XL, Cardilate MR, Coracten SR, Coracten XL, Fortipine LA 40, Hypolar Retard 20, Nifedipress MR, Nifopress Retard, Tensipine MR

Nifedipine treats high blood pressure (hypertension), chronic stable angina (chest pain brought on by physical effort), and vasospastic angina (chest pain caused by spasm of the coronary arteries, sometimes called Prinzmetal's angina). Lowering blood pressure with nifedipine helps reduce the risk of strokes and heart attacks.

The capsule form is approved for vasospastic angina and chronic stable angina only — it is not approved for high blood pressure. The extended-release tablet (including Procardia) covers all three uses: vasospastic angina, chronic stable angina, and high blood pressure.
