Why this applies to Theophylline
Theophylline falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Sulforaphane interacts with.
Animal and in vitro research shows that sulforaphane inhibits CYP1A2 enzyme activity. However, its precursor glucoraphanin also appears to increase the expression of the CYP1A2 enzyme, which could lead to increased metabolism of CYP1A2 substrates. These effects have not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the situation with theophylline and sulforaphane. Theophylline is cleared from your body mainly by a liver enzyme called CYP1A2, and it's a drug we have to keep in a fairly narrow window. Too little and your breathing isn't well controlled; too much and you can feel nausea, a racing or pounding heartbeat, shakiness, trouble sleeping, or in serious cases seizures.
The tricky part is the research points in two directions. In lab and animal studies, sulforaphane can block CYP1A2 (which could let theophylline build up), while its broccoli precursor may speed up that enzyme (which could lower your levels). None of this has actually been shown in people, so it's theoretical. Eating broccoli is almost certainly fine; a concentrated supplement is worth running by your pharmacist or doctor first.

Theophylline — available as extended-release capsules (Theo-24), extended-release tablets, and oral solution (Elixophyllin) — is used to treat symptoms and reversible airflow obstruction caused by chronic asthma, emphysema, and chronic bronchitis.
All supported formulations share these indications. Theophylline is intended for patients with ongoing or recurring symptoms, not for sudden (acute) breathing attacks, which need faster-acting treatments.
