Why this applies to Verapamil

Verapamil falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Sulforaphane interacts with.

Theoretically, sulforaphane might alter the levels and clinical effects of CYP1A2 substrates.
Animal and in vitro research shows that sulforaphane inhibits CYP1A2 enzyme activity. However, its precursor glucoraphanin also appears to increase the expression of the CYP1A2 enzyme, which could lead to increased metabolism of CYP1A2 substrates. These effects have not been reported in humans.

Here's the honest picture with verapamil and sulforaphane (the compound concentrated from broccoli and other cruciferous veggies). On paper, sulforaphane could nudge the activity of a liver enzyme called CYP1A2, which is one pathway that helps process verapamil. The tricky part is the direction isn't clear: in lab and animal studies sulforaphane blocks this enzyme, while a related compound it comes from may actually speed it up. So we can't confidently say verapamil levels would go up or down.

If levels rose, you might notice more dizziness, low blood pressure, a slow pulse, or constipation. If they dropped, blood pressure or heart rate control could slip. The reassuring news is this is theoretical and hasn't been seen in people, and food amounts of broccoli are very unlikely to matter. Still, before starting a concentrated supplement, run it by your pharmacist or doctor.

Theoretically, sulforaphane might increase the levels and effects of CYP3A4 substrates.

Here's the gist for verapamil. Your liver uses an enzyme called CYP3A4 to break verapamil down. In lab and animal studies, sulforaphane appears to slow that same enzyme. If that held true in people, verapamil could clear more slowly and build up a bit, which might nudge its effects stronger. With verapamil, that could mean a slower heart rate, lower blood pressure, dizziness, tiredness, or swelling in the ankles.

The important part: this has only been seen in test tubes and animals, never confirmed in humans, so it's theoretical. Normal broccoli on your plate is very unlikely to matter. A concentrated sulforaphane supplement is the bigger question mark, so check with your pharmacist or doctor before adding one.

About Verapamil

What Is Verapamil Used For?

Brand names: Isoptin SR, Verelan, Isoptin, Calan SR, Verelan SR, Calan, Covera HS, Verap, Veramil

Verapamil tablets and film-coated tablets are used to treat angina — including vasospastic (Prinzmetal's) angina, unstable angina, and chronic stable angina — as well as certain abnormal heart rhythms (arrhythmias) and high blood pressure (essential hypertension). For arrhythmias, regular-release tablets are used alongside digoxin to control heart rate in atrial flutter or atrial fibrillation, and to help prevent episodes of paroxysmal supraventricular tachycardia (SVT), a type of rapid heartbeat.

Extended-release and sustained-release oral forms (tablets and capsules) are indicated specifically for high blood pressure. Verapamil hydrochloride injection is used in hospital settings for rapid conversion of SVT to normal rhythm and for temporary control of fast heart rates in atrial flutter or atrial fibrillation.
