Why this applies to Tezacaftor, Ivacaftor

Tezacaftor, Ivacaftor falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Hops interacts with.

Theoretically, hops extract might alter metabolism of CYP3A4 substrates; however, this effect may not be clinically significant.
Animal research suggests that specific constituents of hops, called lupulones, can induce hepatic CYP3A4 enzyme activity. However, a pharmacokinetic study in healthy postmenopausal patients with normal metabolism shows that taking a standardized extract of spent hops containing prenylated phenols, as 59.5 mg twice daily for 2 weeks, decreases the concentration of alprazolam, a CYP3A4 probe substrate, by 7.6%. This reduction is unlikely to be clinically relevant.

Both tezacaftor and ivacaftor (in Symdeko) are cleared from your body mainly by a liver enzyme called CYP3A4. The thought here is that hops might gently nudge that enzyme to work a little faster, which could in theory lower the level of these medicines in your blood and make them slightly less effective at helping your CFTR protein work the way it should.

The good news is this concern is very mild and mostly theoretical. It comes from lab and animal studies, and a human study using a hops extract only dropped a similar test drug's level by about 8 percent, which is too small to matter. Still, since Symdeko is a precise cystic fibrosis treatment, I'd mention any hops supplement to your CF team or pharmacist before starting it, just so everyone is on the same page.

What Is Tezacaftor and Ivacaftor Used For?

Brand name: Symdeko

SYMDEKO (tezacaftor/ivacaftor) is used to treat cystic fibrosis (CF) in patients aged 6 years and older. To qualify, a patient must either have two copies of the F508del mutation, or at least one mutation in the CFTR gene that lab data or clinical evidence shows will respond to this combination.

If a patient's genetic mutation is not already known, an FDA-cleared CF genetic test is recommended to confirm whether they have a mutation that responds to SYMDEKO.
