Treprostinil (IV) falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Fucus Vesiculosus interacts with.
In vitro evidence suggests that a constituent of Fucus vesiculosus, known as fucoidan, has anticoagulant effects. However, in clinical research, fucoidan does not seem to have significant anticoagulant activity when taken orally, possibly due to poor absorption.
Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..
Here's the gist for treprostinil (Remodulin), which you receive through an IV. Besides widening blood vessels in your lungs, treprostinil can mildly affect platelets and slightly raise your bleeding tendency. The thought here is that bladderwrack contains a compound called fucoidan that, in test tube studies, acts a bit like a blood thinner. Stacked on top of treprostinil, the worry is slightly easier bruising or bleeding.
In real life, though, this concern is mostly theoretical. When people swallow fucoidan, the body barely absorbs it, so it hasn't shown meaningful blood-thinning effects in human studies. I still wouldn't start bladderwrack without checking with your care team first, especially since its iodine can also throw off your thyroid.

CYTOCHROME P450 2C8 (CYP2C8) SUBSTRATES

In vitro research shows that fucoidan, a constituent of Fucus vesiculosus, inhibits CYP2C8. This interaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for Remodulin (treprostinil). Your liver uses an enzyme called CYP2C8 to help clear this medication. Lab studies suggest that fucoidan, one of the compounds in bladderwrack, may slow that enzyme down a bit. In theory, that could let treprostinil build up slightly, which might mean stronger blood-vessel effects like headache, flushing, jaw pain, nausea, diarrhea, or low blood pressure.
I want to be honest about the evidence here: this is only test-tube research, and it has never actually been seen in people taking the two together, so the real-world risk looks small. Still, IV treprostinil is a powerful, closely-dosed drug, so don't add bladderwrack without first checking with your doctor or pharmacist.

What Is Treprostinil Injection Used For?
Brand name: Remodulin
Treprostinil injection is used to treat pulmonary arterial hypertension (PAH, WHO Group 1) — high blood pressure in the arteries that supply the lungs. It works to reduce the symptoms that make exercise difficult. Clinical studies included patients with moderate-to-severe PAH symptoms (NYHA Class II–IV) due to idiopathic or heritable PAH, PAH related to congenital heart defects, and PAH related to connective tissue diseases.
It is also used for PAH patients who need to transition off epoprostenol — a related medication — to slow the worsening of their condition during that switch. The risks and benefits of the transition should be carefully discussed with a specialist before starting.
