Asciminib falls under ORAL DRUGS — the group Glucomannan interacts with.

Theoretically, glucomannan may decrease absorption of drugs taken orally.
Due to its viscosity and bulking effects, there is concern that glucomannan can decrease the absorption of oral drugs. A small clinical study in healthy volunteers shows that taking glyburide 2.5 mg plus glucomannan 3.9 grams with breakfast reduces plasma levels of glyburide when compared with breakfast and glyburide alone. In addition, animal research demonstrates this effect on amoxicillin, but shows increased absorption of metronidazole. This mouse model also demonstrates that metronidazole elimination is prolonged, but amoxicillin elimination is enhanced by 38%; glucomannan may also affect the distribution of some drugs. To avoid changes in absorption, take glucomannan 30-60 minutes after taking oral drugs.

Here's the basic concern with pairing glucomannan and Scemblix (asciminib). Glucomannan is a thick, gel-forming fiber, and that gel can physically trap medicine in your gut and slow or reduce how much gets absorbed. Asciminib is not a prodrug, so it doesn't need activating. The worry here is the opposite problem: if less of it gets into your bloodstream, your leukemia treatment could be less effective over time.

Honestly, this is mostly a timing issue rather than a reason to avoid fiber. The evidence comes from a small study with a different drug and some animal data, so it's more theoretical for asciminib. A safe habit is to separate them, taking glucomannan about 30 to 60 minutes after your asciminib dose. Because this is a cancer medicine, please confirm the spacing with your oncology pharmacist or doctor before starting any fiber supplement.

Scemblix (asciminib) is used to treat adult patients with Philadelphia chromosome-positive chronic myeloid leukemia in chronic phase (Ph+ CML-CP) — a type of blood cancer driven by an abnormal protein called BCR-ABL1. It covers three groups: people newly diagnosed with Ph+ CML-CP, people who have already been treated for Ph+ CML-CP, and people whose cancer carries a specific mutation called T315I that makes it harder to treat.

The use in newly diagnosed Ph+ CML-CP was approved under accelerated approval based on early response data. Continued approval for that specific use may depend on results from confirmatory trials confirming long-term clinical benefit.
