Why this applies to Azacitidine

Azacitidine falls under ORAL DRUGS — the group Glucomannan interacts with.

Theoretically, glucomannan may decrease absorption of drugs taken orally.
Due to its viscosity and bulking effects, there is concern that glucomannan can decrease the absorption of oral drugs. A small clinical study in healthy volunteers shows that taking glyburide 2.5 mg plus glucomannan 3.9 grams with breakfast reduces plasma levels of glyburide when compared with breakfast and glyburide alone. In addition, animal research demonstrates this effect on amoxicillin, but shows increased absorption of metronidazole. This mouse model also demonstrates that metronidazole elimination is prolonged, but amoxicillin elimination is enhanced by 38%; glucomannan may also affect the distribution of some drugs. To avoid changes in absorption, take glucomannan 30-60 minutes after taking oral drugs.

Here's the thing to know if you take the oral form of azacitidine (Onureg): glucomannan is a thick soluble fiber that turns into a gel in your gut. That gel can physically wrap around pills and slow or block how much of a medicine actually gets absorbed into your bloodstream. With a cancer drug like azacitidine, getting less than your full dose absorbed could mean it doesn't work as well as it should, which really matters when the goal is keeping your leukemia in remission.

This concern comes mostly from studies on other oral drugs, so it's a reasonable theory rather than something proven with azacitidine itself. To be safe, a simple spacing trick helps: take your azacitidine first, then wait 30 to 60 minutes before the glucomannan. Please check with your oncology team or pharmacist before combining them, and never adjust your azacitidine on your own.

It is generally well tolerated when taken with plenty of water, but it must be taken carefully because it can swell and cause choking or blockage if not swallowed properly. About Azacitidine

What Is Azacitidine Used For?

Brand names: Vidaza, Onureg

Azacitidine injection (brands: Vidaza and Azacitidine) is used to treat adults with several subtypes of myelodysplastic syndromes (MDS) — a group of bone marrow disorders where blood cells don't form or work properly. Vidaza is also approved to treat infants and children 1 month and older who are newly diagnosed with juvenile myelomonocytic leukemia (JMML), a rare blood cancer affecting young children.

Onureg (the oral tablet form) is used differently: it is for continued treatment of adults with acute myeloid leukemia (AML) who achieved remission after intensive chemotherapy but are not able to continue intensive treatment. These are distinct uses — Onureg cannot be swapped for the injectable forms, and vice versa.
