Why this applies to Filgrastim

Filgrastim falls under ORAL DRUGS — the group Glucomannan interacts with.

Theoretically, glucomannan may decrease absorption of drugs taken orally.
Due to its viscosity and bulking effects, there is concern that glucomannan can decrease the absorption of oral drugs. A small clinical study in healthy volunteers shows that taking glyburide 2.5 mg plus glucomannan 3.9 grams with breakfast reduces plasma levels of glyburide when compared with breakfast and glyburide alone. In addition, animal research demonstrates this effect on amoxicillin, but shows increased absorption of metronidazole. This mouse model also demonstrates that metronidazole elimination is prolonged, but amoxicillin elimination is enhanced by 38%; glucomannan may also affect the distribution of some drugs. To avoid changes in absorption, take glucomannan 30-60 minutes after taking oral drugs.

Here's some reassuring news. The concern with glucomannan is that its thick, gel-like fiber can grab onto medicines in your stomach and slow how much gets absorbed. That only matters for pills and liquids you swallow. Filgrastim (Neupogen) isn't taken by mouth, it's given as an injection under the skin or into a vein, so it never passes through your gut where glucomannan does its work.

Because of that, this particular interaction really doesn't apply to your filgrastim shots. The fiber won't change how well your white blood cell counts respond. That said, if you take any oral medicines, separate them from glucomannan by taking the fiber 30 to 60 minutes later. And since you're on cancer treatment, it's always smart to run any new supplement past your oncology team or pharmacist first.

Glucomannan is a soluble fiber from the konjac root that swells into a gel in your stomach, which may help with mild weight loss, constipation, and modestly lowering cholesterol. It is generally well tolerated when taken with plenty of water, but it must be taken carefully because it can swell and cause choking or blockage if not swallowed properly. About Filgrastim Injection

What Is Filgrastim Injection Used For?

Brand name: Neupogen

Filgrastim injection (available as Neupogen, Nivestym, and Releuko) is used to lower the risk of serious infection in people whose white blood cell counts drop dangerously low. This can happen during chemotherapy for cancer, during treatment for acute myeloid leukemia (AML), or after bone marrow transplantation. It is also used to help move stem cells from the bone marrow into the bloodstream so they can be collected for transplantation.

In addition, filgrastim is used long-term to reduce infections in people with severe chronic neutropenia — including congenital neutropenia, cyclic neutropenia, and idiopathic neutropenia. Neupogen is also indicated to increase survival in patients acutely exposed to radiation doses that suppress the bone marrow (Hematopoietic Syndrome of Acute Radiation Syndrome).
