Why this applies to Lomustine

Lomustine falls under ORAL DRUGS — the group Glucomannan interacts with.

Theoretically, glucomannan may decrease absorption of drugs taken orally.
Due to its viscosity and bulking effects, there is concern that glucomannan can decrease the absorption of oral drugs. A small clinical study in healthy volunteers shows that taking glyburide 2.5 mg plus glucomannan 3.9 grams with breakfast reduces plasma levels of glyburide when compared with breakfast and glyburide alone. In addition, animal research demonstrates this effect on amoxicillin, but shows increased absorption of metronidazole. This mouse model also demonstrates that metronidazole elimination is prolonged, but amoxicillin elimination is enhanced by 38%; glucomannan may also affect the distribution of some drugs. To avoid changes in absorption, take glucomannan 30-60 minutes after taking oral drugs.

Here's the thing about glucomannan: it's a thick, gel-forming fiber that sits in your gut and can grab onto medications you swallow, slowing or reducing how much gets absorbed. With Lomustine (Ceenu), an oral chemotherapy capsule, that's a real concern. If the fiber blunts how much lomustine your body takes in, you could end up with less drug reaching your bloodstream, which for a cancer medicine means it may not work as well against the tumor.

This is mostly a theoretical and timing-based worry rather than proven for lomustine specifically, but with a drug this important I'd play it safe. Don't change anything on your own. Please talk with your oncology team or pharmacist before pairing these, since spacing them apart or avoiding the fiber may be the right call.

Glucomannan is a soluble fiber from the konjac root that swells into a gel in your stomach, which may help with mild weight loss, constipation, and modestly lowering cholesterol. Lomustine treats primary and metastatic brain tumors (tumors that start in the brain or spread there). It is used after appropriate surgery and/or radiation.

It is also used with other chemotherapy drugs for Hodgkin's lymphoma that has gotten worse after first-line chemotherapy. Gleostine and the Lomustine capsule products are all labeled for these same uses.
