Why this applies to Midostaurin

Midostaurin falls under ORAL DRUGS — the group Glucomannan interacts with.

Theoretically, glucomannan may decrease absorption of drugs taken orally.
Due to its viscosity and bulking effects, there is concern that glucomannan can decrease the absorption of oral drugs. A small clinical study in healthy volunteers shows that taking glyburide 2.5 mg plus glucomannan 3.9 grams with breakfast reduces plasma levels of glyburide when compared with breakfast and glyburide alone. In addition, animal research demonstrates this effect on amoxicillin, but shows increased absorption of metronidazole. This mouse model also demonstrates that metronidazole elimination is prolonged, but amoxicillin elimination is enhanced by 38%; glucomannan may also affect the distribution of some drugs. To avoid changes in absorption, take glucomannan 30-60 minutes after taking oral drugs.

Here's the simple version: glucomannan is a soluble fiber that turns into a thick gel in your gut. That gel can wrap around a pill and slow or reduce how much of it gets absorbed into your bloodstream. Since midostaurin (Rydapt) is taken by mouth, the worry is that taking it too close to your fiber could mean you absorb less of your leukemia or mastocytosis medicine, which could make it work less well.

This is mostly a timing concern, and the evidence here is limited (it comes from other oral drugs, not midostaurin itself). The easy fix is separating them. A good rule of thumb is to take your midostaurin first and wait 30 to 60 minutes before any glucomannan. Because this is a cancer medicine, please run any fiber supplement past your oncology team or pharmacist before starting.

What Is Midostaurin Used For?

  • Midostaurin is used with other chemotherapy drugs to treat certain types of acute myeloid leukemia (AML; a type of cancer of the white blood cells). Midostaurin is also used to certain types of mastocytosis (a blood disorder in which there are too many mast cells [a certain kind of white blood cell]). Midostaurin is in a class of medications called kinase inhibitors. It works by blocking the action of the abnormal protein that signals cancer cells to multiply. This helps stop the spread of mast and cancer cells.
