Midostaurin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Berberine interacts with.

Theoretically, berberine might increase serum levels of drugs metabolized by CYP3A4.
In vitro research and preliminary clinical research show that berberine moderately inhibits CYP3A4. Theoretically, use of berberine with drugs metabolized by CYP3A4 might increase drug levels and increase the risk of adverse effects.

Here's the thing to know if you're on midostaurin (Rydapt): your body relies on a liver enzyme called CYP3A4 to break this medicine down and clear it. Berberine appears to slow that same enzyme down a bit. So in theory, adding berberine could let midostaurin build up higher than expected in your blood, which may mean more of its side effects, things like nausea, mouth sores, swelling, low blood counts, fatigue, or changes in your heart rhythm (QT prolongation).

I want to be honest about the evidence: this concern is largely theoretical, based on lab and early human studies, not on people specifically combining these two. Still, midostaurin is a serious chemotherapy drug, so I wouldn't add berberine on your own. Please run it by your oncologist or pharmacist first.

What Is Midostaurin Used For?

Brand name: Rydapt

  • Midostaurin is used with other chemotherapy drugs to treat certain types of acute myeloid leukemia (AML; a type of cancer of the white blood cells). Midostaurin is also used to certain types of mastocytosis (a blood disorder in which there are too many mast cells [a certain kind of white blood cell]). Midostaurin is in a class of medications called kinase inhibitors. It works by blocking the action of the abnormal protein that signals cancer cells to multiply. This helps stop the spread of mast and cancer cells.
