Why this applies to Midostaurin

Midostaurin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cranberry interacts with.

Theoretically, cranberry might increase the levels and adverse effects of CYP3A4 substrates.
A case of upper back pain, rhabdomyolysis, and abnormal liver function has been reported for a patient taking atorvastatin, a CYP3A4 substrate, in combination with cranberry juice 16 ounces daily for 2 weeks. Creatinine kinase and liver enzymes normalized within 2 weeks of stopping cranberry juice. Also, animal research suggests that cranberry juice, administered intraduodenally 30 minutes prior to nifedipine, a CYP3A4 substrate, inhibits nifedipine metabolism and increases the area under the concentration-time curve by 1.6-fold compared to control.

Your body relies on a liver enzyme called CYP3A4 to clear midostaurin out of your system. The thought here is that cranberry, in concentrated amounts, might slow that enzyme down a little. If that happens, midostaurin could build up higher than expected, which in theory could mean more of its usual side effects, things like nausea, diarrhea, mouth sores, fatigue, low blood counts, or changes in your liver enzymes.

I want to be honest about how thin this evidence is. It is mostly theoretical, based on a single case with a different drug (a statin) and some animal data. An occasional glass of cranberry juice is unlikely to cause trouble. Since midostaurin is a serious cancer medicine, please run any daily cranberry juice or supplements by your oncology team or pharmacist first.

What Is Midostaurin Used For?

  • Midostaurin is used with other chemotherapy drugs to treat certain types of acute myeloid leukemia (AML; a type of cancer of the white blood cells). Midostaurin is also used to certain types of mastocytosis (a blood disorder in which there are too many mast cells [a certain kind of white blood cell]). Midostaurin is in a class of medications called kinase inhibitors. It works by blocking the action of the abnormal protein that signals cancer cells to multiply. This helps stop the spread of mast and cancer cells.
