Acalabrutinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Berberine interacts with.

Theoretically, berberine might increase serum levels of drugs metabolized by CYP3A4.
In vitro research and preliminary clinical research show that berberine moderately inhibits CYP3A4. Theoretically, use of berberine with drugs metabolized by CYP3A4 might increase drug levels and increase the risk of adverse effects.

Here's the quick picture: your body relies on a liver enzyme called CYP3A4 to break down and clear acalabrutinib. Berberine appears to slow that same enzyme down. So if you take both, your acalabrutinib could clear more slowly and build up a bit higher than expected. Acalabrutinib is an active drug (not a prodrug), so more drug in your system means a stronger effect and a greater chance of side effects like headaches, bruising or bleeding, low blood counts, infections, or an irregular heartbeat.

Honestly, this is mostly a theoretical concern based on lab and early research, but acalabrutinib is a serious cancer medicine, so I wouldn't gamble with it. Please check with your oncologist or pharmacist before adding berberine.

What Is Acalabrutinib Used For?

Brand name: Calquence

Calquence is used to treat adults with mantle cell lymphoma (MCL) — either newly diagnosed (in combination with bendamustine and rituximab, for patients who are not candidates for a stem cell transplant) or previously treated (after at least one prior therapy). It is also used to treat adults with chronic lymphocytic leukemia (CLL) or small lymphocytic lymphoma (SLL).

Both the capsule and tablet forms of Calquence are approved for all three of these uses. The specific combination regimen and treatment schedule depend on the condition being treated and what other medicines are used alongside it — your care team will map that out for you.
