Why this applies to Oliceridine

Oliceridine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Greater Celandine interacts with.

Research in vitro shows that chelidonine, a constituent of greater celandine, inhibits cytochrome P450 2D6 (CYP2D6) enzyme activity. Theoretically, greater celandine might increase levels of drugs metabolized by CYP2D6.
Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), clozapine (Clozaril), codeine, desipramine (Norpramin), donepezil (Aricept), fentanyl (Duragesic), flecainide (Tambocor), fluoxetine (Prozac), meperidine (Demerol), methadone (Dolophine), metoprolol (Lopressor, Toprol XL), olanzapine (Zyprexa), ondansetron (Zofran), tramadol (Ultram), trazodone (Desyrel), and many others.

Oliceridine (Olinvyk) is an IV opioid given for serious short-term pain, usually in a hospital. Your body relies partly on a liver enzyme called CYP2D6 to clear it. Greater celandine contains a compound (chelidonine) that can slow that same enzyme down, at least in lab studies. If celandine blocks CYP2D6, oliceridine could clear more slowly and build up in your system.

For an opioid, that extra buildup matters. It could mean stronger drowsiness, confusion, nausea, very slow breathing, or low blood pressure. Oliceridine itself is not a prodrug, so this points to a stronger effect, not a weaker one. Honestly, this is theoretical and based only on lab data. But since celandine is also tied to liver injury, please clear it with your doctor or pharmacist first.

Olinvyk (oliceridine) injection is approved for adults to manage acute pain that is severe enough to require an intravenous opioid analgesic. It is used only when alternative treatments are ineffective, not tolerated, or otherwise unable to provide adequate pain control.

Because of serious risks including addiction, misuse, overdose, and death, Olinvyk is reserved for situations where other pain management options are not sufficient. It is not intended for routine or chronic pain management.
