Here's the thing to know about guar gum and your bicalutamide (Casodex). Guar gum is a thick, gel-forming fiber, and when you swallow it with a pill it can grab onto that medicine in your gut and slow how much of it gets absorbed. With bicalutamide, which is an active drug you take by mouth (not a prodrug that needs activating), the worry is the opposite of too much: you could end up absorbing a little less, which over time might make it work less well against the prostate cancer.

This is a possible, moderate concern based on limited human data with other oral drugs, not bicalutamide specifically, so don't panic. A simple fix is timing. Take your bicalutamide first, then wait 30 to 60 minutes before the guar gum, and check with your pharmacist or oncologist before combining them.

What Is Bicalutamide Used For?

Brand name: Casodex

Bicalutamide (50 mg daily) — available as the brand Casodex and as generic bicalutamide tablets — is used together with an LHRH analog (a hormone-lowering injection) to treat Stage D2 metastatic carcinoma of the prostate (advanced prostate cancer that has spread to other parts of the body). It is not used alone for this purpose.

Important: The 150 mg daily dose of bicalutamide is not approved for use alone or with any other treatment.
