Here's the simple version. Guar gum is a gel-forming fiber, and when it swells up in your gut it can grab onto medicines you swallow and slow down how much gets absorbed. With Larotrectinib (Vitrakvi), an oral cancer pill, the worry is that less of it reaches your bloodstream. Since this drug works by quieting the abnormal protein driving tumor growth, getting too little could mean it doesn't work as well as it should.

This is a moderate, possible concern based on how guar gum behaves with other oral drugs, not specifically tested with Larotrectinib. A good habit is to separate them: take your Larotrectinib first, then wait 30 to 60 minutes before the guar gum. Please run this by your oncology pharmacist or doctor before adding any fiber supplement.

What Is Larotrectinib Used For?

Brand name: Vitrakvi

Vitrakvi is used to treat adult and pediatric patients with solid tumors that have a neurotrophic receptor tyrosine kinase (NTRK) gene fusion — a specific genetic alteration that can drive tumor growth. It is used when the tumor is metastatic (has spread) or when surgery to remove it would likely cause serious harm, and when there are no satisfactory alternative treatments or the cancer has continued to grow after prior treatment.

Patients must be selected for Vitrakvi therapy based on an FDA-approved test that confirms the NTRK gene fusion in their tumor. This approval is under the FDA's accelerated approval pathway, based on how many patients responded to treatment and how long those responses lasted. Continued approval may depend on results from follow-up clinical trials.
