Aspirin, Butalbital, Caffeine, Codeine Phosphate falls under CNS DEPRESSANTS — the group Hesperidin interacts with.

Theoretically, concomitant use with CNS depressants may cause additive sedative effects.
Animal studies show that hesperidin has sedative effects, due to opioid receptor activity and can increase sedation when used with diazepam. This effect has not been reported in humans.

Here's the simple version: Fiorinal with Codeine already has two ingredients that calm the brain down, the butalbital (a barbiturate) and the codeine (an opioid). On top of that, lab and animal studies suggest hesperidin may have a mild sedative effect of its own, partly by acting on opioid receptors. So in theory, pairing them could add up to extra drowsiness.

The key word is theoretical. This hasn't actually been reported in people, and the evidence comes only from animal research, so I don't want to alarm you. Still, if you notice more sleepiness, dizziness, or feeling foggy, that's worth paying attention to. Before combining them, run it by your pharmacist or doctor so they can look at your full medication list.

Here's the piece that matters with Fiorinal with Codeine: it contains aspirin, which already thins your blood a bit by quieting your platelets (the cells that help you clot). Hesperidin, a citrus flavonoid, may have a mild blood-thinning effect of its own. Put the two together and, in theory, you could bruise more easily or bleed a little longer from a cut, a nosebleed, or your gums. In rarer cases, that can mean stomach bleeding, so watch for black or tarry stools.

I'll be honest with you though: this concern comes from lab and animal studies, not real patients, so it's more theoretical than proven. A serving of orange isn't the worry, concentrated supplements are. Before combining them regularly, run it by your pharmacist or doctor.

Hesperidin is a flavonoid found in citrus fruits that is often combined with diosmin and used for vein and circulation problems like hemorrhoids and varicose veins. Some evidence supports these uses, but data for many other claims is limited. It is generally well tolerated, but pregnant and breastfeeding people should avoid it because safety has not been established.
